What is included in the ADHD Concierge Membership?

The ADHD Concierge Membership includes the following services provided through scheduled ADHD visits:

      • Comprehensive ADHD evaluation and treatment planning
      • Stimulant medication management and ongoing monitoring
      • Annual physical examination
      • Electrocardiogram (EKG), as clinically indicated
      • QB Testing (FDA-cleared computerized neurocognitive evaluation)
      • Completion of documentation for ADHD-related needs (e.g., disability forms, school accommodations, FMLA)
      • Treatment of commonly co-occurring conditions: mild to moderate anxiety, depression, and sleep difficulties (including initial evaluation for sleep apnea)

What are the fees for membership and are there any discounts?

      - Annual fee: $1,488
      - Payment structure: $399 initial payment, followed by $99/month
      - 25% discount for full-time students
      - 15% discount for patients under age 20

Is the membership a substitute for health insurance?

No. This membership is not a substitute for health insurance and only covers ADHD-related care and associated mild/moderate conditions.

Will you bill my insurance for visits or services?

We do not bill insurance directly. However, we will provide a superbill for you to submit to your insurance. If you are a member, we can submit the superbill on your behalf without a processing fee. Non-members will incur a 3% administrative fee for submissions.

Are services outside of ADHD care included in the membership?

No. If, during a covered ADHD visit, you request evaluation or treatment for a condition not included in your membership, the following add-on fees apply:

      • Minor issue (less than 10 min): $40
      • Moderate issue: $75
      • Full second issue / extended care: $120

      Are in-person visits available?

      - Members: Yes, during designated clinic hours at no additional cost.
      - Non-Members: Generally not available unless specifically arranged and may incur added fees.
      - Home visits may be available to both members and non-members for an added fee.

What is your cancellation and no-show policy?

Appointments canceled with less than 24 hours' notice or missed without notice will be subject to a $60 fee. Repeated no-shows or cancellations may result in termination of care.

      How is my privacy protected?

All patient information is protected under HIPAA. Telehealth encounters are conducted via secure, encrypted platforms, and records are only shared with written permission or as required by law.

      Can I cancel my membership?

Yes. Membership may be canceled by either party with 5 days’ written notice. Re-enrollment requires a 2-month wait and repayment of the $399 setup fee.

What if I’m not diagnosed with ADHD after signing up?

The initial membership fee covers the full diagnostic evaluation to determine whether you have ADHD. If you’re not diagnosed, you won’t be obligated to continue with the membership or pay the ongoing monthly fee. You’ll only be responsible for the initial payment, which reflects the cost of the evaluation.

Can I still receive care or remain in the membership without an ADHD diagnosis?

Yes. If you’re not diagnosed but would like to continue care, Dr. Kamen will honor the member-rate pricing for the remainder of your first year. After that year, your rate would revert to standard non-member pricing.

You’re also welcome to remain in the membership. While it’s designed around ADHD care, the membership also includes treatment for related conditions such as mild to moderate anxiety and depression. If your care needs are limited and visits are infrequent, it may not be financially necessary to stay enrolled—paying per visit could be more cost-effective. That said, remaining in the membership guarantees access to as many telehealth visits as needed within the scope of care included in the membership description.
